(as a relative),

We found my frail and vulnerable mother lying in her own urine on the Medical Assessment Unit. The ward was under staffed and in my opinion she was neglected. She died on Appley Ward in view of other patients with no privacy.

I was extremely sorry to read that you and your mother had a very poor experience of our Trust, and extend sincere condolences on the loss of your mother.

As a Trust we would value the opportunity to discuss your experience in more detail, and if you have not already done so would urge you to raise this with us directly via the Quality Team or Chief Executive in order for us to gain more understanding and address these issues raised.
