"Why can't we make the appointment after the visit? "

(as a relative),

My wife has regular check ups as a former cancer patient. Appointments were always made after she had seen the doctor. Nowadays they say they will send a letter for the next appointment. No such appointment was made for her. Why can't we make the appointment after the visit? Then we know its done as check ups are usually 6 months to a year and can be forgotten.
