* A&E Nurse left cannula and IV Drip Bag taps closed... then said "it wasn't working" * Written up for Morphine in A&E... not administered because staff were in a teaching huddle * Admitted to Renal ward... but not seen by doctor for several hours, because they could not decide if I was "Renal" or "Surgical" *No wheelchair or porter available to take me from Ward to CT Scan. We had to find a wheelchair ourselves, and find our own way to CT scan. * When seen by doctor, immediate action was to recommend operation under GA to insert stent. Only with our intervention\enquiry was Lithotripsy considered (cancellation appointment available same afternoon... just didn't ask) * When seen by consultant on Outpatient visit, he was unaware I had undergone Lithotripsy session... records had not been collated\updated.

On behalf of the Emergency Department (ED) we would like to apologise that the nurse did not notice the IV cap was closed and thought it was not working. This is a training issue and the matron for the area has raised this with the ED practice educator to ensure that it is addressed with the nursing team. We are also sorry that the prescribed morphine was not administered in a timely manner, adding to your discomfort. The renal team would also like to apologise that you found the medical and nursing care disjointed during your hospital admission to Victoria Ward. The matron for renal services is very sorry that you and your family felt that you had to intervene to move your care forward across the two specialities and that records were not communicated to your consultant in the outpatients department. We are unsure why the staff were unable to assist with obtaining a wheelchair and having to make your own way to the CT scan without a porter to escort you. This is not our policy and we would like to apologise that this happened. Your feedback is very much appreciated and will be shared with the emergency and renal teams in order to improve patient care for the future.
