"Access to support for Mental Health - Bipolar"

My daughter has suffered from depression for 20 years. Earlier this year she was diagnosed as Bipolar and is on medication. When she is well she is great and she copes with the minor ups and down of the illness but she has now had two big episodes this year to date and I am struggling to get her the support she needs when this happens. The GP Practice and the local Mental Health Provider don't seem able to work together so when my daughter is in crisis I cannot readily get help from either. After numerous phone calls I have now managed to get the local Crisis Team to visit her and the GP has a telephone appointment with her. This has taken several days and a lot of nagging to organise.
Please can you consider the following for Bipolar: An easily accessible single point of access which can provide support, medication and can communicate across primary and secondary care. Access to support for self-help - what to do when she goes high, which is then followed by the massive low; how she can cope with a teenage child; how that teenage child can cope with her; what I can do as her Mum.
