(as a relative),

Mum was discharged from hospital recently and was cared for by the  Hospital@home team who then discharged Mum to Community health District Nurses.

I am writing to praise and thank the District Nursing Team  for their excellent care and attention provided for Mum until her untimely death at the beginning of  October.  The District Nurse team made a very difficult situation easier for our family with their knowledge, care and kindness which was very much appreciated. With kind regards and appreciation.

Dear Carmen Obre (Senior Nurse)

Thank you for your response and feedback regarding our NHS experience. Mum lived in Hamilton Lanarkshire.

As requested I confirm that the Hospital @home team ( Ashley, Lou & Barry) were based at Monklands Hospital North Lanarkshire.

The District Nurse team were based at Udston Hospital Hamilton Lanarkshire
