"Frailty assessment unit- Great practice, care and…"

This review is based on the frailty assessment unit which is where my mum was seen today, unfortunately there is no section for the frailty team, can this department be added to the 'review of services section' so that the fabulous care, friendliness, and attention to detail can be recognised. All staff were so kind to my mum and tried to put her at ease. She had a full MDT assessment and the team all worked so well together. It was all summarised then by the consultant, ACP and therapy and my mum went away with a really clear healthcare plan. Please can this review be passed on to the frailty team to acknowledge the extremely high level of care that was delivered. Its such a fabulous service with the aim of keeping people out of hospital.
