"Inaccurate and incomplete discharge summary"

This is about my wife, who was recently discharged from Clarendon Wing, LGI, Leeds. Following an operation for her broken clavicle. they were late in producing a discharge summary and when they did it was badly inaccurate and missing important date. This was things like the operation, the drugs she was put on, the follow up appointment with both the surgeon and the physiotherapist and the request to our GP to prescribe more painkillers if necessary, all of these were missing. So as far as our practice is concerned they have been not given any relevant details only that she visited A & E.
