"Awesome service and care"

(as a relative),

There are no words to describe how awesome and thankful I was to have this amazing service available for my Dad at home whose health was declining very quickly from Parkinsons/Dementia. A team of private and care agency staff looked after dad at home. The day I had to go in for major surgery Dad's health nose dived and the carers contacted his GP on my behalf, who was always thorough and a great help when contacted.

She very promptly got the Home Hospital Team out for my Dad. They assessed and administered medication daily until I got out of hospital 5days later. Dr Irena of the team kept me informed while I was in hospital. She is one of the most friendly, warm, caring and empathic doctors I have ever come across. She and the team consultant spoke with me the day after I came out hospital about Dad and very gently helped me to understand that there was no more that could be done for Dad and it was now an 'end of life' situation. Dad passed away quietly at home about 10days later.

Thank you to all involved within the Team, several I never got to meet, you were all amazing. Thanks also to the Rapid Response Team in Acute Enniskillen who rang me every evening during those final 10days checking on his condition and immediately available when required, as was the Marie Curie nurses when called upon when Dad got anywhere near stressed. The district nurses were very attentive and brilliant throughout. 

Altogether an amazing collective effort by everyone to ensure the best for my dad in his final days. He was comfortable and in no pain until his last breath. I pray that God may bless you all and keep each of you. May His face shine on you and may you know His peace. The only way I can bless you for the blessing you were to me and Dad.

I am sorry to hear that your father has sadly passed away and my thoughts are with you at this time.

Thank you for your feedback story.

Community Care Services are vital, particularly for people living with dementia, to ensure continuity of care is maintained in their home environment, without any distressing and unnecessary transfers to hospital.

Person centred care is paramount within dementia care and this has been evidenced throughout your experience. Approach and communication are vital in the caring profession, not only to ensure that all your father’s needs were met, but also that your needs were met during your difficult situation at that time.

End of life care can be complex and can involve a number of different professionals across one or more services. I am equally delighted to hear that the care and treatment provided was of high standard throughout, compassionate, empathetic and person centred for both you and your father, during this critical aspect of care delivery for someone living with dementia.

Firstly on behalf of the Hospital at Home team can we offer our sincere sympathies on the passing of your father. Our thoughts are with you and your family at this sad time.

Thank you for your kind comments. We are so glad you were happy with the care your Dad received and also the support you got through daily communication/updates within the Hospital at home team whilst you yourself where in hospital.

Dr Carp is very grateful for your kind comments concerning her care for your Dad.

It is our goal as a team to give excellent patient centred care and we are so glad to hear your Dad was comfortable and cared for in a dignified manner in his final days in the comfort of the place he called home.

We trust that you are recovering after your recent surgery also and thank you again for taking the time to leave such positive feedback.
