I was admitted to hospital via my GP due to a flare up of my Inflammatory Bowel Disease. Unfortunately my own care team were not available at this time.
A clear plan of care was implemented one day & the next day I was reviewed by someone different within Combined Assessment Unit (CAU) and felt as though I was treated unfairly. There was no holistic care provided, no awareness of my rare health condition & no option to explain it either.
My discharge letter contains incorrect information & I'm extremely disappointed with how I have been left to feel as a patient. I appreciate there are bed pressures within hospitals, but this is no excuse for sub par care.
It was also highlight by a healthcare professional that my Lateral Flow Test (LFT) for COVID-19 should've been carried out on admission. There was also conflicting information during my short admission about what form my medication should be prescribed in.
