ICATT supported the transition for a very complex young person from the family home into a supported living placement. Both ICATT teams were involved throughout the transition as this person moved from the North of the county into a placement in Nottingham.
From our perspective, the input from both ICATT teams has been so professional, timely and at times over and above what could be expected. One example is an OT supporting the providers to put together flat pack furniture on the day of the move so that the environment was more suitable for the person when they arrived and to minimise any risks or avoidable behavioural outbursts. Very much appreciated by the provider too I should imagine.
The team have supported with delivery of training, updating of PBS plan and have offered valuable advice and support to ensure the care/support plans are a true reflection of the person's needs. Their input has also supported quality monitoring and safeguarding enquiries to be completed.
I am sure that this transition would not have been successful if it had not been for the support given to the provider, family and wider MDT (including the CCG), and this should be formally recognised. Many thanks in particular to Emma, Steph, Adam, Chris and Gareth for their support with this transition.
