Story by wife of 70-79 year old male patient from Derry/Londonderry area

This is the second time we have had Acute care at home.  Great friendly team, great support and provides great medical care in the home.  Thank you all so much.

I also see you have received treatment from the team on more than one occasion. One of the aims of the team is to prevent admission to hospital which clearly happened in your case.
