Thank you so much for sharing your visual story of the Condition Management Programme we are delighted you had a positive experience of the Programme and experienced been seen as a whole person, listened to and supported in planning for the future in relation to living with Post Covid Syndrome.

We have shared your story with the team which is greatly appreciated and your story will provide encouragement for other people to hopefully access the Programme.

We also appreciate the areas of improvement you highlighted included more timely, dietary education and multi-disciplinary care. We are a work focused short-medium term Programme providing up to twelve sessions, funded by the Department for Communities to support people to enter, return to or remain in work by supporting them to understand and self management their health conditions. For the last nine months we have experienced a significant increase in demand for the service which has resulted in waiting times for the Programme to increase. The Department for Communities receive a copy of all the Care Opinion stories which in this case demonstrates the experience of timely access requiring improvement. Thank you for raising these issues as a Programme we strive to ensure we are continuously improving to meet our clients’ needs within the Programme remit.
