"Request from works occupational health ignored"

About: Riverside Resource Centre

(as a service user),

Over 7 months ago, my employers occupational health service contacted a doctor at Riverside Resource Centre for a report on my recent autism diagnosis and fitness for work. This has been chased on several occasions by my Mother, the HR dept and Occupational Health. I have now been informed in the last 4 weeks that due to failure of the doctor to respond they cannot continue to employ me and at my next attendance review they have no option but to terminate my employment.

My mother has spoken to the doctor's secretary several times who simply states it has not been dictated to them or put on their desk and therefore the report has not been done. 

I am confused as to why it has not been done.  They have been made aware via the secretary of the urgency of this for 7 months. My employers have simply nowhere else to go now without this report.

The meeting is tomorrow. Its too late and my employment termination will lead to more damage to my fragile mental health and self harm. I am off on sickness leave as I struggled to cope with changes due to covid restrictions. I am devastated as my job gave me a reason to continue being a person who had a purpose.
