"Unsatisfactory discharge of patient"

About: North Devon District Hospital / Cancer Services

(as a relative),

My husband was discharged on the Saturday of a Bank Holiday weekend after nearly a month in hospital following a lymphoma diagnosis, Covid infection and heart problems. No medical team member spoke to me about his condition or what to expect. We were given no discharge sheet and all his medications, both current and discontinued ones, were put in a bag together, with no information about what he was taking, when and what dosages.

He left with an open pressure sore but no dressings, treatment or advice for it. We were given no details for anyone to contact for support as I take on a full caring role due to his condition. We both feel very let down and if it wasn’t for friends in the medical profession, we would have been in real difficulties.

Dear Lynbird,

I am so sorry to hear about your husband's discharge and that you were not provided with the relevant information. I am sorry you feel let down and I would like to reassure you that we value feedback to make improvements.

Firstly I would like to check and make sure that you and your husband are ok, so it would be really helpful if you could get in touch so that we can look into this further and establish which ward he was discharged from, and also your contact details so we can provide any help that you need now with his dressings or any supportive information.

Could you contact the patient experience team by email and a member of my team will be in touch. Our email is ndht.patientexperience@nhs.net

Thank you

Teresa
