"Care plan related to previous falls on ward 51"

I posted a story about how poor care and communication led to my being injured in falls and how a nurse had told me I was “independent”. I have since requested my notes and discovered what might be at the root of this experience.

I was admitted to hospital as a result of three falls at home and being in a state of confusion. I was subsequently diagnosed with a urine infection. This is documented in the hospital’s  admission notes.

However on my personal care plan across the topics of “mobility” and ‘personal hygiene” is scrawled “INDEPENDENT”. “Falls risk” is described as ‘LOW RISK”. This is repeated on each daily care plan until after my first fall. Interestingly on the heading of the care plan “Discussed with patient on...” is left blank. 

For me this raises a huge question as to what information this care plan was based on.

I have fed this back to the ward manager and she assured me this would be discussed with staff. I include it here for the benefit of the people who read my original post.

Feedback whether positive or negative involving my ward practice is extremely valuable and taken seriously.

As ward manager I have had several conversations with the patient which have been useful and we agreed that going forward I would ensure I discussed their points with my team and they would be addressed and actioned.

I would like to assure you I have done just that and asked my staff to reflect on their practice.

We have discussed this at length but in light of new information in the way care plans are generated I felt it would be useful for others who have viewed the post to be kept in the loop.
