"The maternity assessment unit let me down"

About: Maternity care / Maternity Assesment/Triage

(as a service user),

The maternity assessment unit let me down and I believe their mistakes caused my still birth at 41 weeks of a healthy pregnancy.

They failed to bring me in despite me telling them my temperature was low, they should have recognised this as a sign of sepsis. I later went in to septic shock and am lucky to be alive. I’m told the midwife team are trained on recognising low temperature as a sign of sepsis, however have also been told that the two midwives I spoke to clearly made a mistake.

There were also other failures in my care, too many to list here unfortunately. The hospital has admitted this through the investigation. I'm promised that improvements will be made however we will have to see. My consultant was brilliant, but I have to be honest and write this post.

It would be hard for me to rely on the midwives here after my experience, at least not when calling the maternity assessment unit. The phone line seems to not be managed or monitored very well and in my experience it seems too easy for mistakes to be made and incorrect advice to be given, then the hospital to apologise later for something as huge as a stillbirth, and events that could have been life-threatening.

Dear Harinder

Many thanks for taking the time to share your experience on care opinion at what has been a very distressing and difficult time for you and your family and I would wish to offer my sincere condolences on the very sad loss of your baby.

Within Maternity Service in Greater Glasgow and Clyde we strive to provide safe and effective woman centred care and from what you describe you clearly have been let down and for that we sincerely apologise.

As an organisation we always wish to learn from feedback so that we can improve the experience for women and their families and I was glad to read that you found your consultant supportive.

I would like to reassure you that where we identify any areas of improvement from feedback or any local investigations, then these are reviewed and monitored through our clinical governance systems.

I would like to offer you the opportunity to make contact with us so that we can provide you with feedback on the improvements that were discussed with you and if you wish to take us up on this offer please contact Associate Chief Midwife Mairi McDermid at Mairi.McDermid@ggc.scot.nhs.uk

Kind wishes

Evelyn Frame
