"Acute Care at home"

About: Rapid Response Teams / Rapid response team Derry/ Londonderry

(as a staff member posting for a carer/relative),

Story by daughter of 80+ year old female patient from Derry/Londonderry area

The care was patient directed and every member of the team treated mammy in a friendly, compassionate manner.

It made such a difference that she could be treated at home in her own bed rather than having to go to hospital.  We consider ourselves very lucky to be able to avail of this home care service for mammy.  Thank you all very much.

Thank you very much for your feedback

Hopefully there comes day when people don't feel lucky to avail of a service such as ours but that this type of service becomes common place for Older People.

When possible Home is the best place to be cared for in a familiar environment and with loved ones.

I am very happy that our care was delivered in a person centered manner we work so hard to achieve this.

your feed back will be shared with the team.

Regards Sr O Hara
