"Records appear to have been misplaced"

Important records relating to my care, appear to have been misplaced. This is obviously concerning, as I assumed there would be something in place to prevent it from happening. 

I am also curious to know why it is, that whilst all stories published on this site get read, but not all stories have a response. Please could you explain?

I'm James from Care Opinion and I wanted to respond to the question you raise about responses.

Care Opinion is a non-profit organisation and not part of the NHS, so we have no power to ensure that stories shared here are either read or responded to. We have spent many years trying to persuade healthcare organisations that this important.

As a result, many stories (but not all) are read, and many (70-80%) are responded to. In some organisations, and in some parts of the UK, response rates are much higher. In Scotland and Northern Ireland Care Opinion has been adopted by the system, and response rates are 98% or more.

In England, it is variable. You can see the response rate for an organisation on the relevant service page, or compare response rates on our live chart.
