"Terrible practice"

We are trying to switch to another GP. I have had numerous negative experiences with this practive over the last several years. Just a few examples: Hygeine: my child had a vaccine where the nurse dropped the needle on the floor as she was unwrapping it (so it was partly exposed) and then jabbed my child with it. Failure to address urgent medical needs: my son's asthma has taken a turn for the worse in the last few weeks and we tried to have a consultation/review of meds etc. We spoke to the receptionist who told us she could not give us an appointment but rather we must fill out a lengthy online form and then someone would call us back. After doing so, a few days later (!) a 'nurse' from the practise called to discuss his case and refused to help since he is young (4 years old) and so according to her he cannot have a change in treatment from his existing meds (!). After being turned away by this 'nurse' we called the receptionist again to again trying and get an appointment with a doctor and were again told we must go and fill in the same online form. It has been about a week of daily asthma attacks and trying to get an appointment for my 4 year old son, and still after much back-and-forth we have no prospect of help in sight. Communication generally: they often do not answer the phone but if you do get through, the receptionist seems to be actively unhelpful (see above). Another current example: the practise did not follow up the results of tests - we have had to chase them as my wife is suffering severe kidney pain this past week and they did some tests and failed to follow up with us on the results.
