"They pointed me in the right direction"

My grandfather spent 3 months in the hospital and all he wanted for the last bit of time he has left was to come home and be with his wife and go outside in the fresh air. The social prescribing team made this possible! Without them my grandad would have no quality of life. They put me in the right direction for everything. All my grandad wanted was to go outside and feel the air on his face and have a wander round his local shops! To do this we needed a wheelchair for him to which 2 applications had failed due to him being end of life care. The social prescribing team helped me and did there up past best to point me in the right direction and it paid off as my grandad now has a quality of life and can get outside for a walk. Without teams like this I fear those of us fighting for our relatives but not knowing how would be a lost cause! I’m so grateful to this team as is my grandfather and cannot thank them enough!
