(as a relative),

My husband who has Chronic Kidney Disease, was referred to the team on the evening of 14th march. Next morning we had a home visit from Jaqueline who explained in detail the remit of the team and backed this up  with printouts of tbe stages of his illness. She also confirmed he wanted to stay at home with the famiy. Blood tests were taken daily and results back in less than 2 hours, she returned to explain these and kept us fully informed. 

Sadly these have all led to confirm he has started on the end of life path. 

I cannot praise the team,Drs Din and Flatt, Jaqueline and Kerry  , for the care and compassion given to us both over a very stressful week. 

He has now been passed back to GP and district nurses
