"Discharge from CMHT"

About: Tiverton Hospital / Silverlea Community Mental Health Team

(as a service user),

I feel my discharge from CMHT has not been a positive experience which is very important to me having had many bad experiences of endings in the past which have impacted me greatly. The care coordinator informed me of my discharge but alas there has been no goodbye meeting or telephone call this I feel is a totally unacceptable way to to treat a patient.

Thank you for taking the time to provide feedback about the service you received from Trust. I am very sorry your discharge was not a more positive experience. Your comments suggest you were discharged with limited communication from the team and no discharge meeting, which may have been as a result of COVID, but there should have been some discussion with you about this.
