"Covid questionnaire"

About: NHS Tayside

(as the patient),

I was asked to complete a Covid questionnaire before a medical appointment. I tried to access the Covid questionnaire but there is no sign of a questionnaire. This in no way detracts from the quality of care I experienced following an accident in October 2020.
