"Denied Covid vaccination"

My Father aged 89 and in the top priority group for Covid 19 Vaccination was denied vaccine yesterday as he could not sign the consent form.

Yet, he was referred to the "vaccination hub" at Croydon University Hospital straight after a outpatient appointment at the hospital with an Elderly Care Consultant.  

Why is there no procedure in place for the consent forms to be signed by a carer or doctor in the case of a patient not being deemed able to sign.

Why was the referring member of staff not aware of the procedure at the vaccination hub.

My Father will now have to return at a later date to get his vaccine once the procedure for him getting the vaccine is communicated to him / his carer . It is unclear who will explain the procedure and how the next vaccination appointment will be communicated to him.

What a waste of an opportunity to vaccinate an elderly vulnerable person who was on site and ready to get the vaccine.   

Every time he leaves the house for a hospital appointment it puts him at risk, I am very disappointed that the vaccination team were unable to carry out the vaccination.

I am his next of kin and live overseas.  I have a LPOA which I messaged to his carer.  The NHS system of contacting patients and making appointments is very difficult for the elderly and very hard for me to keep up with being out of the UK at this time.
