Last October I was referred to ACH for investigations into ongoing illness I have and subsequently I attended ACH in December 2019 during which blood samples and chest x-ray were taken. A follow up appointment was duly received and attended in mid January where additional bloods were taken along with a referral for a pulmonary function test at UHA which I attended in mid February.
Its now been seven months since the last ACH appointment and to date I have not received any further correspondence as to whether I require further follow ups or tests from ACH.
Given that my illness continues to be an issue and with the COVID restrictions lifted, I attended my local GP surgery where they confirmed that they were also waiting for feedback from ACH.
Failure to follow up patient care in a timely and appropriate manner has the potential for missed investigations, undiagnosed illness, investigations not being followed-up or referral to appropriate specialist. Ultimately this has lead to poor patient care and could have additional impact on patient health.
