"Lack of care plan. No care in the community"

About: Ysbyty Ystrad Fawr / Mental health care

(as the patient),

Met doctor in February 2020.

My CPN left her job.

I was trying to get support to help me with online prescriptions repeats. Unfortunately I still haven't had no care in the community which I am entitled to.

My phone calls over 3 months have been ignored I had 1 phone call telling me no one was around and another email will be sent.

I have acute paranoid symptoms which are getting worse I think they are doing it on purpose and want me to die.

They know I am alone without any relatives family or friends that can help when I am un well.

It's getting beyond a joke and my care at the moment is clozapine clinic at ystrad mynach every four weeks with a blood nurse may I add very unfamiliar faces. I asked for help and the nurse said she is not I.T literiture and of old skool material and couldn't help. She dismissed  me and gave my closapine and sent me out next one in.

It's getting to the point now I am looking at funeral plans and organising my LPA (legal power of attony).

I am entitled to this care and advice but because I ain't dead or carving my pain upon my own body I ain't going to get help to prevent my schizophrenia  affecting me in the community.

What is a patient in this cliche suppose to do
