"Key information on discharge from hospital"

About: St Michael's Hospital (Hayle)

(as a service user),

Just prior to discharge from hospital; I was given key information.  In context, the key information given to me was around drugs which had been prescribed.  I was told that whilst 'x' drugs had been annotated to have been for 14 days; it was incumbent upon me to get in touch with my GP as  I was only given circa 4 to 5 days supply.  The drugs were key to pain control.

Given that at the time under the confines of COVID - visiting was not permissible.  This is fully accepted however it wasn't until I was at home on the Sunday that the conversation came back to me about the drugs.  Luckily, my GP was able to prescribe on the day both the drugs up to and including the allotted amount.

I just thought I would feedback that for key information, perhaps a telephone call to next of kin and/or note - stating to be opened once a home - to ensure key information is noted when the patient is not fully able to take in and/or remember what is being said.

Hi defenderrq53,


I just wanted to update you on where we are with the making an improvement to our service following your feedback.

As each persons situation is different we are now discussing with each individual on admission about what their preference is for passing on information for discharge. This is then documented as part of their discharge care plan and we then know who to speak to and about what. This way, we know that the information that we are sharing has been agreed in advance and that the process isn't rushed.

Hopefully this will help people in future!
