Following my last two Care Opinion stories which have not been authorised yet, I wanted to request my daughter's Medical records but NHS Tayside won't send me them. We have written and applied by the official form but no reply and no records. We have had a letter to say that the diagnostic notes provided by our private hospital, are now part of my daughter's clinical record and cannot be released to us.

Without more detail it is difficult to respond on your individual concerns, however there are clear processes within NHS Tayside to obtain access to your medical records via our Medical Records Department:
Access Office
Health Records Department
Level 7
Ninewells Hospital

If, as you state you have already made your application and are unhappy with the decision then you can ask the health professional involved to explain how the decision was made and what information was used. If you wish further information regarding the decision please contact:

The notes arrived today. Clearly there was a backlog of work at medical records. We did have to request it several times.
