"First midwife appointment"

About: Nottingham University Hospitals NHS Trust - City Campus / Maternity care Nottingham University Hospitals NHS Trust - Queen's Medical Centre Campus / Maternity care The University of Nottingham Health Service

(as a service user),

When I was barely able to see the line on my first pregnancy test in November 2019, I called my medical practice, Cripps Medical Centre. They asked me to do another test in a week's time, after which I called again. From then on, every time, they told me to wait another week or two, and finally gave me another telephone number to call, without explanation. It turned out that this was Radford Medical Practice, the central place for making a first midwife appointment in Nottingham. I provided my contact details and never heard back. I called and called, having many problems getting through to a midwife, and then again, being told to wait another week or two, and that they will call me back. At week 13, they had me 'on the radar' and by week 14, things were very urgent and everything suddenly had to happen. I had just started a new job, not told anyone at work about the pregnancy yet and I don't drive, making same-day visits on the other side of the city in the middle of the day quite difficult. I felt like a failure, with the indirect messages, also coming from midwives and nurses, that the baby was not important to me. I was terrified of losing my job, as I was in the middle of a four-week probationary period, although, as mentioned above, I had called the medical practices as early as possible. I was suddenly swamped with appointments all over the city, during the day, with the longest lasting 3 hours, which I had to work around. It was very stressful.   I did then have a community midwife appointment local to where I live.

I don't know what went wrong, as everyone I told this story has been surprised. I felt not cared about, including the baby. At the worst points, I considered abortion, because I felt utterly powerless to do my job as a mother, while at the same time facing the accusations for it. The problem was that I had no idea, how pregnancy works in the healthcare system - and no one to ask! The messages from the medical practices were contradictory (call later / call the other number / we don't do pregnancy tests - how should I know??)

Once I got into the system, my first scan was arranged quickly for NUH things improved and the healthcare professionals there and at Queens were very caring, friendly and helpful. It took a while, before the situation relaxed.  I also passed my probation and my colleagues have been very supportive, which is kind of a happy end to the story.

We are very sorry to hear that you are unhappy with the service you received from our practice. Whilst we are unable to comment on the service you received from the midwife team based at Radford Medical Practice, we can certainly look into what happened in more detail in regards to when you called us at the very early stages of your pregnancy. In order for us to get a bit more information from you, please contact the practice at your convenience and ask to speak with the practice manager. We can then investigate this fully. Thank you for providing us with your feedback, we take all feedback extremely seriously and always endeavour to improve our service where necessary. We are also very pleased to hear that things improved for you and hope the rest of your pregnancy progressed without issue.

Thank you for taking the time to get in touch with us. I’m sorry to hear that the communication and experience you received didn’t match your expectations and that the service you received caused you to worry unnecessarily at what is already an anxious time. I am pleased to hear that once you were referred to NUH you felt cared for and supported but if you would like to contact us to discuss any aspect of the care you received during your pregnancy please get in touch by emailing QMCPET@nuh.nhs.uk or call 0115 924 9924 ext 86623.

Kind regards

Thank you for your response, Anthony Harbottle. Saying that the communication and experience I received didn’t match my "expectations" does not quite capture the problem, as, firstly, expectation setting was the initial problem and, secondly, the midwives, nurses and doctors had expected me to attend appointments before week 13/14 and some were quite surprised that I was in the system that late. Would you see a problem with their expectations as well? I would also not say that I worried unnecessarily. To be fair, there were times, when I was quite relaxed and thought "Okay, I'll just wait a few more weeks." The urgency arose when I was picked up by the system, as I needed to have my first scan before the end of week 14, which I only got because my pregnancy was re-dated. It turned out I should have taken a certain supplement during my first trimester because of a pre-existing health condition. Obviously, I completely missed that.
