"Poor discharge"

About: University Hospital Monklands / Care of the Elderly (Wards 14,20,22)

(as a relative),

Mum was discharged from ward 14 UHM where she had been admitted following a fall.

Very poor discharge instructions, poor follow up instruction and no dressings for district nurses. Personally contacted district nurses who hadn’t received the referral (on a Friday).

This is the second bad experience we've had from Monklands and I worry for people who don’t have anyone to be the advocate they need!

I'm so sorry to hear about you and your mum's recent discharge experience. This is clearly not the standard of care and communication that we aspire to and I can only apologise for that.
