"Coordinated support across health care services"

(as a staff member posting for a carer/relative),

Dear Hilary, Anna, all at St Luke’s Single Point of Access (SPA),  all at St Luke’s day services, Alison, Dr Holland, Dr Phoebe and everyone else who looked after my mother during her recent illness.

Please do pass this on to anyone at St Luke’s who I have missed.  I will also send some thank you cards, again, hoping they will be shown to everyone who looked after my mother.

I would like to thank everyone who supported my father, Mark,  and looked after my mother, Helen, during her recent illness. You probably know that she died in January at home. I am sorry it has taken so long to thank you, but practicalities have taken up a lot of time.

She received the most amazing care from the NHS in her last few months, but particularly in the last week. There was always a nurse available to answer the SPA phone line, ready to give excellent advice, or to contact an appropriate service to help my mother.   She had support from St Luke’s day services, the Community Palliative Care team, Dr Bleehen (GP at the Pinn Medical Centre), a visit from an out of hours GP, the  district nurses, and  the ambulance service.  There was also a very kind and helpful private lymphoedema nurse, who visited her at home, and the Caremark agency (not NHS), who provided some really caring carers. This was the NHS at its best!

Even after my mother had died, you were all so caring and helpful, taking time to talk to all of us, even when we felt your time might be better spent with those who were ill.

Once again, thank you so much for all your help and support over the last few months, and just to let you know that we are all coping well.  I am sure this is largely due to the fact  that her last few weeks and days were  peaceful and comfortable due to the amazing service we received from you all.

Dear Sir

Thank you for taking time to give positive feedback demonstrating the coordination between different health professionals.

Single Point of Access service is a telephone advice and our constant effort is to minimise pressure of patient and their families in crucial time. We will liaise with GP and other health professionals who are involved in the care for best outcome. We understand that for a family member is so difficult to understand different dynamics of everyone role and to remember their numbers, so we can make calls behalf of you and signpost.

I am glad that we were able to do well as a team. I have passed your feedback to the team. Once again thank you very much.
