"Failed discharge"

(as a relative),

My mum late eighties  was in hospital after a fall and confusion. Social worker asked me if i was happy for her to go back home. I said no, she was still too frail. I don't live near (2hour drive away). Despite that, hospital said they needed her bed and she was classed medically fit. She was sent home with minimal care package, and reablement team supplement. They rang me on their visit, saying they were worried about mum being left overnight on her own. To avoid taking her upstairs  they made a bed on the sofa.

4am, her lifeline company phoned, saying she'd fallen and paramedics called. I phoned mum and she said she was on the floor  cold and wet herself. I rang 999 3 times trying to get someone to her. Took 4 hours. Sent back to hospital, traumatised. She never went back home. She is in a care home and i have to sell the house to pay for mum and Dad's care.
