"Issues with discharge procedure"

The hospital is friendly and welcoming. The nurses and care assistance excellent, the lady in reception very helpful. But there is a major issue with discharging patients. A day is picked for discharge and no matter how the patient is on the day or even the day before it seems they will be discharged on the preordained day.

A major mistake was made on ordering home oxygen and clerical error was blamed. When a patient's well being is on the line I feel there should be NO mistakes. The discharge letter lists an illness for the patient that they do not actually have, another mistake.

Tablets were never discussed with patients caregiver, just thrown into a bag to be transported home, if you do not know the dose and the boxes are all the same colour. They need a new and more competent discharge policy and procedure.

Thank you for taking the time to share your feedback with us and for the lovely compliments regarding the staff at South Molton Community Hospital (SMCH).

I am sorry to learn of the issues you raise with regards to the discharge arrangements for patients. I have liaised with the Ward Manager of Hugh Squier ward at SMCH and she is very sorry for the experience that both you and the patient had regarding discharge and medication. The Ward Manager is grateful for your comments and advises that she will discuss them with her team to review how the errors which occurred can be prevented in future.

The Ward Manager would be happy to have a conversation with you to understand in greater detail the events which took place. Please can you contact the SMCH on 01769 572164 and ask to speak with the Ward Manager of Hugh Squier ward.
