My father was admitted to A+E in December 2019, he was admitted overnight and was transferred to AMU. My father is 86 and is unable to walk or stand alone, so is cared for at home with carers and his wife, he is hoisted to bed/chair.
He was able to return home on the 24th December after I called to ask when he could be discharged.
The doctor kindly called me to explain they were happy for him to return home that day, I explained he would need transport to get him home but if push came to shove (as it was xmas eve) myself and my partner could get him into a wheelchair by physical lifting him into the wheelchair and car ourselves but could the doctor please arrange transport for him, and get back to us as to whether or not he was coming home on transport.
After hearing nothing for 2 hours I called the ward to see if he had been booked on transport to come home, they told me that they were told I was collecting him myself.
So at 6pm on xmas eve myself, my partner and my son (dad is still a tall man and quite heavy) went to AMU and physically lifted him into a wheelchair and got him into the car to return him home.
Did anyone on the ward read his notes? If they did they would see he could not stand or walk so I believe should have ensured his return on transport.
I find this to be unacceptable care of a frail and elderly patient.
