"GP Surgery Experiences"

At my GP surgery I have experienced incorrect entries in notes, not always accurate record of appointment, and assumptions of how I felt.

Referral letters being incorrect, saying issues recent when they have been over many years, that they had not pattern when they very very clearly did. Medication prescribed without considering suitability, represcribed even though it did not work previously, no discussing side effects when there is a contradictory medical history.

Taking years to be taken seriously on conditions which I now find could have been treated/managed a long time ago. GP saying they would do something but made no entry in notes to say they did. GPs not communicating with each other. No consistency, having to waste a lot of my appointment time updating GP I am seeing each time for ongoing issues.

All bar one, the nurses are excellent!
