About: Forth Valley Royal Hospital / Oral and maxillofacial surgery

(as a service user),

I got a letter telling me I have an appointment on ? at ?.

No date or time but it says if I don't turn up I may be removed from the waiting list. 

I'm already stressed about this being a local anaesthetic. 

And the letter I got last week asking for any dates I'm not available had the wrong phone number on it.

Doesn't give me any confidence 

I've been told this procedure is necessary as the area could develop into cancer in the future. 

At this point I feel I'd like to be referred to a different hospital if that is possible.

Many thanks for taking the time to get in touch, I am so sorry that the letter you were sent did not have all the information, this must be very distressing for you at what must already be a very worrying time.
