"My notes were confused with another person"

About: East Surrey Hospital

(as the patient),

My notes were confused with another person and only because of my vigilance was this brought to light. Further to this following a CT scan at the hospital my notes were misread and if not for my persistence, I almost had an operation I did not need.

I am extremely disappointed with the level of staff . I spent 14 days as an inpatient and to reiterate, hugely, hugely appalled and disappointed.
