A letter from a rheumatology specialist nurse was sent to an occupational therapist, a physiotherapist and the GP practice. I, as a patient, only saw the letter when I attended these three people. It contained an account of the consultation that I had with the specialist nurse which did not reflect the content of the consultation, so much so that I wondered if they had muddled me with someone else. The areas I had pain and the problems I was having with daily function were incorrect. If I had received a copy myself, I could have corrected these and not had to give a fresh history to the health professionals, wasting their time and mine.
