"Being discharged from Pyrland Ward"

About: Somerset NHS Foundation Trust / Older people's community mental health

(as the patient),

I was discharged from Pyrland Ward, I was very happy with my treatment there and also with the discharge process. Just one small point, the discharge papers do not mention that my husband, who is also my carer, was present. All the other people who were present  are detailed but not my husband, I wondered why?

Hello there,

Thank you so much for taking the time to let us know about your treatment with our team at Pyrland Ward - I am so pleased to hear that this went well for you whikst you were there and that you were well supported through the discharge process That truly is great to hear as we know that discharge from services can be a worrying time for lots of us.

I have shared your story with Jo, the deputy ward manager at Pyrland who was delighted to hear that you were so happy with your care and she will be sharing this with the whole team. Jo has also asked me to pass on her sincere apologies that your husband was not mentioned on your discharge paperwork and to reassure you that this was not intentional and was an oversight on the team's part and they are very sorry about that. Please do get in touch directly with the ward and they can certainly amend the discharge paperwork for you and resend to you and whoever needs to receive a copy.

Jo will also take this opportunity to remind the ward team about the importance of including relatives, carers and family members in all discharge plans and we are grateful that you have taken the time to share this with us and that we can make some changes to make the experience even better!
