"Chronic pain not understood"

About: East Midlands Ambulance Service NHS Trust / Emergency ambulance Nottingham University Hospitals NHS Trust - Queen's Medical Centre Campus / Accident and emergency

(as a service user),

I was taken to A and E by ambulance after vomiting 10 times in 3 hours, shaking, severe head pain, a suspected head injury and unable to tolerate fluids.

I suffer with chronic pain issues so had attempted to manage my pain, unsuccessfully, at home throughout the day, however, eventually had to call 111 for advice who sent an ambulance. I was treated with care and respect by the ambulance crew but felt badly treated by staff in ED. Due to some of my regular medications being 'controlled drugs' I was advised to leave these at home.

Despite informing various members of staff throughout my stay in ED about my severe pain and requesting pain relief I was consistently ignored. Prior to my admission i had been attempting to take strong painkillers on an hourly basis but vomiting them straight back.

After 5 hours in the department, despite vomiting, and having a cannula I was reluctantly offered paracetamol/ibuprofen by a nurse. After this I wretched for over an hour. Due to the delay in the department I had missed my evening dose of medication so became even more uncomfortable due to the withdrawal from my usual medications.  Despite trying to explain this to members of staff and requesting further pain relief, I was again ignored. I was incredibly/painfully uncomfortable throughout my whole stay but felt I was consistently ignored.

I was eventually seen by a doctor who was very polite and concise who then decided that I needed to speak to the Department of Psychological Medicine. However, I am aware of my diagnosis or reason for my symptoms and deterioration in my health.

I was eventually seen by DPM and discharged home from there.

On getting home I had still not passed urine (for over 10 hours despite a bladder condition) and had a cannula in my arm.

Throughout my whole stay in the department I did not eat or drink anything, and wretched on water with the medication. Since discharge I have still been unable to eat anything and only managed 500ml in 24 hours which i do not consider a safe discharge.

I once again feel that as I suffer with chronic pain, my concerns regarding my pain level were ignored, dismissed and not taken seriously. Because of my coping strategies to manage this chronic pain I was once again denied appropriate analgesia to manage my condition and presentation to ED.
