"Post hospital discharge planning"

About: Adult services / District nursing St James's University Hospital

(as the patient),

I was discharged from St. James hopital at the end of April. Full details of my community care were faxed that day to Community Nursing.

By 5pm the following day no-one had contacted me to make an appointment to call. I left a message for the team and two nurses came out from the twilight team. They could do nothing about my wound dressing as they did not have any information about me atall. I asked them to telll the day team that I would be at hospital the following morning.

Whilst out on Friday a nurse came and went away again. She did not come back.

Now I must wait until today, Sunday. Again no appointment time has been given and some three days after discharge I am still waiting for a nursing plan.

I feel this is a disgraceful service and does not match up with any goals set.

Thank you for taking the time to share your story on Patient Opinion. I was sorry to hear about the problems you experienced with your care following your discharge from hospital.

I would like to re-assure you that work is ongoing to improve the discharge planning process. A process has been implemented following feedback from patients and carers which includes the hospital sending discharge information to a patient's GP within 24 hours. The discharge team at the hospital are also working with ward staff to improve the discharge procedures. We also hold a monthly meeting to discuss any broken processes and the concern you raised has been taken to this group.
