What could be improved

I accompanied a young person who was a suicide risk. We waited for 4 hours in A&E for the

on call psych team to eventually be told that messages had not been passed on and the team had gone to another hospital. This is completely unacceptable. I understand that resources are tight and a wait was inevitable. However a failure in communication occurred that could have had life threatening consequences .I would like this investigated and a way to be found of ensuring this doesn't happen in the future.

At no point did any of the staff who passed us ,ask if we were ok or use their own initiative to investigate why, having been triaged at 7.40 and told we would be seen at 9.o'clock we were still waiting at 11.45pm.
