What could be improved
If each department talked to each and feedback was actually given to relatives of elderly patients rather than having to keep asking.
Notes should all be correct - not have 3 different dates of birth noted within the folder for my mother.
I am disgusted that my 82 year old mother was eventually discharged without advising anyone she was being sent out. The ambulance arrived at the care home but could only tell them he had a resident for them - did not know her name. There were no discharge notes sent or any advice on care given etc to the care home or relatives. Despite making in excess of 12 calls from the family and the care home, we could not find anyone on the wards to even discuss the care given and confirm if her medication should be continued as before - very worrying as she was complaining of headache and stomach pains within a couple of hours of return therefore did not even know if painkillers were appropriate. Left a call on the complaints hotline and hour and a half ago and not even a call from that team.
Very disspointed at lack of apparent care for a patient.
