doctor on A12 was exceptional
What could be improved
comments on behalf of my mum: was on ward (MAU)a few hours when we noticed the suction and oxygen tubing was dirty and had not been changed since the last patient. patient transferred to A12 within a few days and the same thing happened again. nasal cannula brown at end and dark secretions in tubing, potentialy this could have been used in an emergency and infection passed on.
Also nutritional risk assessment done inaccurately resulting in inadequate care
Also nurse (A12) did not know about home situation although reassured that MDT referral would be done on admission (MAU). bed area could have been better prepared ie new suction and oxygen on wall
requested MDT referral should be done on admssion if requested
potential breakdown in communication when patient transferred from ward to ward
nutritional risk tool is not just a paper exercise but should be accurate and determine appropriate level of monitoring and interventions
Anything else?
I have noticed many things that are wrong in relation to 2 admissions (re mum) in past few months. Have let things go but continue to be disgusted by lack of hygiene and nutritional care
