My father (age 84) was sent to Chester le Street from Durham hospital to stabilise his blood pressure. He was in Chester le Street for two weeks at which time very little (if anything) was discussed with relatives unless we asked.

We were in the process of having a stair lift installed as we felt it was unsafe to come home due to dizzy spells and how ill he is.

We were called on the day to say he was being discharged this was without any assessment carried out in his home to see how he could cope and before the chair lift had been installed.

We feel he was discharged too early as within less than 24 hours he was re-admitted to Durham University Hospital after he collapsed.
