What could be improved

I have visited several wards in the R.H.C.H. and I feel that infection control has gone a miss when patients water jugs are changed.The fresh jugs are given out and the old one put back on the clean trolley , this being carried out using the same hand .The trolley contains several jugs of water often being given out to patients with limited hand washing facilities.The wards can be spotlessly clean but this one act causes concern for cross contamination.
