Disjointed services, no joined up thinking. Seeing my daughters skin continually breaking down is so upsetting. Wheelchair moulded seating destroyed due to incontinence. Time after time this has been brought to the attention of Town Head Clinic in Coatbridge.

I see the sleep system which is to aid my daughters position smelling, stained due to leaking of incontinence and having to be washed daily needing replaced also. This could have been easily avoided had there been a joint thinking approach between health/social care!

Keeping skin from breaking down is preventable!

I ask you to visit my daughter have a look at vital equipment needed to ensure her care needs are met.

Seeking resolution is all that is asked.

Why have Social Work and Health integrated if not to meet health outcomes. Does someone need to be in hospital for anyone to take action. Absolutely shocking and heartbreaking to see my daughter's skin like this.

Collaboration between community support Services would ensure resolution is sought!

Health Social Care Integration surely is needs led?

A Nurses attitude that "she is paying " for another Nurses Advisor is a solution to ensuring the right product is sourced regardless how complex the patients needs are.
