What could be improved

my mother in law was a patient recently in 6Y. She received next to no notice that she was being discharged and I only found out at 3.45pm on friday that she was to be sent home that night without any additional support being in place. She has mental health issues that are made worse by the chest problem that hospitalised her. We were treated like idiots by the social care assessor who seemed to doubt the existence of my mother in law's mental health case worker who had said that home care would be in place before she was discharged and had actually spoken to the nursing staff on the two occasions they had visited. We felt very patronised by the attitude towards us. I am the only local person able to care for her until 'home from hospital' is set up. Because of childcare needs I was unable to pick my mother in law up from hospital when she was finally discharged at 8pm after being given back her usual medications. Because of the complete lack of notice of her being discharged she therefore had to get a taxi home dressed in her pyjamas and slippers (and the rain was pouring down). There seemed to be a total disregard for her wellbeing at the point of discharge.
