"Communication failure"

About: Chesterfield Royal Hospital / Cardiology

I had a pulmonary embolism on 25 December and was admitted by ambulance. I was well served with medicines etc and prescribed a CTScan the same evening to confirm the diagnosis and extent of the problem.

The Scan was delayed until Friday 30 December in spite of my very frequent requests. The problem was subsequently found to be due to the Scan department assuming I had been discharged as they could not find me in the emergency ward.The net result was that I spent 5 days on the ward unnecessarily with the stress of chasing up what I knew was needed but was not available for some unknown reason. The serious concerns I have are a) How could the scan people assume I had been discharged - did they not check the central records? b) why did the ward personnel fail to remedy the problem during 4 days of continual reminders allowing me to become increasingly stressed about the failure to continue the treatment, the noisy, public and frustratingly dis organised ward and trying up a bed unnecessarily ?

Thank you for your post about your stay in hospital over Christmas.

I hope you are checking in and see my response as you have some very specific questions that I am sure our division would want to answer.

You clearly were left with a poor impression of the hospital and your experience of care and treatment and I am sure you would wish to know why it was presumed you had been discharged.
