"Lost blood test and X-ray"

My mother who has terminal cancer and heart failure has taken a nose dive in the last three weeks. In order for her to have a week in the hospice we were told she should first have a check up at the GP. We did this and he said he thought mum may be severely anaemic and probably needs a transfusion and that her right lung did not appear to be working properly. So off we went to st Margarets Hospital Epping for blood test and X-ray. She is in a wheelchair and needs oxeygen and this means such a trip is a major undertaking. This was over a week ago. I was present when the bloods were done and also when the X-ray was done. We made a follow up appointment at the GP today who told us he blood test has gone missing and the X-ray is not on the system though there is evidence it was done. Mum has to go back to the GP for another blood test tomorrow and wait another 48 hours for the result. All this is extending the length of time a very poorly woman has to wait for urgent treatment. It is so frustrating especially because she is so ill and because of the huge effort to get her to appointments. How many other tests were lost that day? Who took the tests? Were they sent to the lab? Is the lab at fault?
